At Jyoti Diagnostic, we are committed to providing safe, reliable, and convenient home sample collection services. By requesting a home sample collection through our website or by contacting us, you acknowledge that you have read, understood, and agreed to the following terms.
1. Voluntary Consent
By scheduling a home sample collection, you voluntarily authorize Jyoti Diagnostic and its authorized sample collection personnel to visit your provided address for the purpose of collecting the required laboratory samples.
2. Accuracy of Information
You agree to provide accurate and complete information, including:
- Full Name
- Phone Number
- Email Address
- Age
- Gender
- Residential Address
Incorrect or incomplete information may result in delays, rescheduling, or cancellation of the appointment.
3. Appointment Availability
Patients or an authorized representative should be available at the scheduled address during the agreed appointment time.
If no one is available, the appointment may need to be rescheduled based on our operational availability.
4. Pre-Test Instructions
Certain laboratory tests require specific preparations, such as fasting or avoiding certain medications.
It is the patient’s responsibility to follow any instructions provided by Jyoti Diagnostic or the referring healthcare professional before sample collection.
Failure to follow these instructions may affect the accuracy of test results.
5. Safe Collection Environment
Patients are requested to ensure that the sample collection area is safe, clean, and accessible for our collection staff.
Jyoti Diagnostic reserves the right to postpone or decline sample collection if the environment poses a risk to the safety of our personnel.
6. Sample Collection Procedure
Our trained sample collection professional will:
- Verify the patient’s identity.
- Collect the required sample(s) using standard medical procedures.
- Follow appropriate hygiene and safety protocols.
- Handle and transport samples according to laboratory guidelines.
7. Rescheduling or Cancellation
Home sample collection may be rescheduled due to:
- Patient unavailability
- Incorrect address or contact information
- Severe weather conditions
- Medical emergencies
- Operational constraints
- Other unforeseen circumstances
Jyoti Diagnostic will make reasonable efforts to arrange an alternative appointment where possible.
8. Patient Responsibilities
The patient agrees to:
- Provide truthful and accurate information.
- Cooperate during the sample collection process.
- Inform the sample collection personnel of any relevant medical conditions that may affect the collection procedure.
- Follow all instructions provided before and during sample collection.
9. Limitation of Liability
Jyoti Diagnostic shall not be held responsible for delays, rescheduling, or inability to provide home sample collection due to circumstances beyond our reasonable control, including incorrect information provided by the patient, restricted access to the premises, natural disasters, transportation disruptions, or other unforeseen events.
